Note: If you are opening a claim under any of the following coverage types, please choose either Option#2 or Option#3 below:

• Lost, Stolen or Damaged Baggage
• Delayed Baggage or Flight Delay (excluding claims on Trip Cancellation/Trip Interruption policies)
• Prescription Glasses Replacement
• Rental Car Physical Damage Protection

This option allows you to print, complete and mail your claim forms directly to OneWorld Assist.
